NEW PATIENT POLICY:

Here at HaleStorm we require all patients to place a card on file to book any service. All new patients to Kayla Hale, PA-C are required to pay a $250 non-refundable booking fee. Deposits apply as a credit towards a future service by Kayla only. New patients of Kayla Hale PA-C cannot reschedule or cancel their initial appointment. In the event you must reschedule or cancel, you may not rebook with Kayla Hale PA-C. For future appointments, you may book with another provider.

CANCELLATION POLICY:

All providers require 48 business hours when cancelling or rescheduling. If you cancel within this window of time, you will owe 50% of the service. If you no-call/no-show, you will owe 100% of the service. Any deposits previously paid will be applied to the invoice. There is a $100 same day cancellation fee for Neurotoxin.

LATE POLICY:

If you are more than 10 minutes late to your appointment, we reserve the right to reschedule the appointment. This policy is in place so we can ensure the providers have enough time to provide a quality service to you. We understand situations arise, and we ask that you communicate with us to ensure courtesy to not only our staff but other patients.

PAYMENT OPTIONS:

We accept cash, check, and all major credit cards. Payment is due at the time of your service. All credit card transactions will be charged a fee of 3% of the total invoice. For financing options, we offer Cherry Financing payment plans. We are happy to provide you with a link to apply. There is a minimum requirement of $1,000 to use Cherry Financing and a 10% processing fee.

GIFT CERTIFICATES/REFUNDS:

All gift certificates must be used within 1 year of purchase. All sales are final. Products are only eligible for a refund within 7 days of the purchase.  If the product does not meet your expectations, we are happy to refund in full, as long as it is within 7 days of your purchase
